No matter what size your plan is, we can provide you with Support Coordination and Specialist Support Coordination services.
Our coordination services are provided through our coordinating_connexions team. Our team have years of industry experience, qualifications in community-based disability and mental health support and skills across a variety of areas (lived and learnt).
What we do for you
Support Coordination helps you to understand and use you NDIS plan to achieve your goals, connect you with the right service providers in the right areas, and build your confidence and capacity in using your plan funding.
We don’t just provide information, we:
- set up the services for you
- ensure your providers know how to meet your needs
- regularly check in with you to review outcomes against your NDIS plan – at least every 3 months
- review our work with you, and ask you what works and what needs to change
- make any plan changes required to ensure you have what is needed
We assist people of all ages, including children, and work across metro Adelaide and surrounding areas.

- In home and community support
- Therapeutic support
- Transport
- Workplace support
