At Coordinating Connexions, we support you to find the services you need to make a difference in your life.
At Coordinating Connexions, we meet with you and together work out how we can maximise your NDIS plan. We set up the services for you, ensure your providers know how to meet your needs, and regularly check in with you to review outcomes against your NDIS plan – at least every 3 months. We review our work with you, and ask you what works and what needs to change.
Our support coordination is provided through our Coordinating Connexions team, which includes staff with skills and experience in social work, mental health (lived and learnt) and community services.
We assist people of all ages, including children, and work across metro Adelaide and surrounding areas.
